One of the things being proposed was a "public option", which would've been basically a government-run insurance option, which would have competed with the existing companies but without spending money on advertising campaigns or worried about generating profits. But the Obama admin never really fought that hard for it and then they abandoned the idea completely once they lost their super majority in the senate. What they ended up with was what Tim said, plus now there is a mandate that Americans HAVE to buy insurance. I don't think true universal healthcare was ever really on the table in the first place.
