Teeth Fair condition
Received is the body of a well developed, well nourished black male appearing compatible with the stated age. The body is received unclothed and no personal effects accompany the body. The head is normally formed and atraumatic. The neck is also intact and atraumatic. The chest, abdomen, and posterior aspects are appropriately developed. The
genitalia are those of a normal adult male. There is no evidence to or discharge from the genitals or anus. The upper and lower extremities are appropriately developed. Evidence of medical intervention include a nasogastric tube, endotracheal tube, a port in the right upper chest, a bandaged needle puncture mark on the left upper chest and a 9"

sutured stapled surgical incision on the midline of the abdomen. Also present is an arterial line in the left wrist and a urinary catheter.

INJURIES
A single stab wound is identified to the left lateral side 22" from the top of the head and 7" to the left of midline. The wound is gaping measuring 0.7" x 0.3" in greatest diameters. The knife is found to enter the body at this site, traveling roughly downwards with subsequent perforation of the left lower lung lobe, diaphragm, stomach, colon and kidney. Internal examination shows surgical intervention including sutures on all the previously named anatomic sites.
Reactive changes are also noted around the left lower lung lobe. The abdomen is found to contain approximately 800 cc of bloody fluid.

In addition to the above, multiple minor scabbed over lesions and minor contusions which may represent defensive injuries are found running from essentially the left biceps to the left wrist. These measuring up to 5" in greatest diameter.

DISPOSITION OF CLOTHING AND PERSONAL EFFECTS

INTERNAL EXAMINATION
Body Cavities
The body is opened with the usual Y-shaped incision using the existing surgical incision. The chest plate is removed. No significant hemorrhage is noted beneath the chest. The perforation from the stab wound is found to be at the 7th intercostal space. The chest cavities combined contain approximately 300 cc of clear yellow fluid. The pericardial sac is intact. The abdominal cavity except as noted above is also intact.

Cardiovascular System
Heart Weight 310 grams
The great vessels arise and distribute normally. The epicardium is red-brown and firm. The coronary arteries are sectioned and are found to be free of significant atherosclerotic occlusion. The ventricles are sectioned and are found to be free of old or recent infarction. No thrombus is found within the atria, ventricles, or appendages. The valves are intact and lined by the usual number of unremarkable leaflets. The aorta and vena cava are free of significant disease process.

Respiratory System
Right Lung Weight 820 grams
Left Lung Weight 970 grams
Examination of the pleural surfaces of the lungs show them to be generally gray-tan and glistening. The pleural surface of the left lung is covered with fibrin. Sectioning through the lungs show consolidation of left lower lung lobe secondary to old hemorrhage. Otherwise, the surfaces are red-brown and free of focal lesion. The tracheobronchial tree including the larynx is intact without evidence of abnormality. A pulmonary embolism is not identified.

Gastrointestinal System
The GI tract is intact except as noted. Sutures are identified around the fundus of the stomach as well as the sigmoid flexure of the colon. A normal appendix is present. The stomach is empty.

Liver
Liver Weight 2310 grams
The capsule is intact and the angles are sharp. On section, the red brown surfaces are unremakable with out evidence of cirrhosis or neoplasia.

Spleen
Spleen Weight 340 grams
The capsule is intact and the angles are sharp. Suture material is not identified at this time. On sectioning, the red-brown surfaces are unremarkable.

Pancreas
The pancreas is of normal size, consistency and otherwise unremarkable.

Urinary
Right Kidney Weight 180 grams
Left Kidney Weight 200 grams
The capsules strip easily. Approximately 10-20 gm of organizing blood is identified beneath the upper pole of the left kidney secondary to the knife wound. Otherwise, the cortical surfaces of both kidneys are red-brown and smooth. On sectioning, the renal architecture is maintained throughout and is otherwise unremarkable. The ureters are intact and drain to an empty bladder (urinary catheter).

Reproductive
The prostate is unremarkable in size and on section.

Endocrine
Both the thyroid gland and adrenal glands are intact and otherwise unremarkable.

Neurologic
Brain Weight 1280 grams
The leptomeninges are thin and delicate. There is no evidence of epidural, subdural or subarachnoid hemorrhage. The cerebral hemispheres are symmetrical and otherwise unremarkable. No significant atherosclerotic occlusion or aneurysm formation is found within the basal artery or circle of Willis. On section, the ventricles are filled with clear cerebrospinal fluid and otherwise intact. No significant abnormality is noted on section.

Musculoskeletal System
Intact except as previously noted. The hyoid bone is not fractured.

MICROSCOPIC EXAMINATION
Microscopic Comment
The slides are reviewed. Except for the findings noted below, the sampled tissues are noncontributory.

Respiratory
Section of lung from the stab wound shows organizing blood.

Genitourinary
Section of kidney from the stab wound shows organizing blood.

SUMMARY AND INTERPRETATION
This 46-year-old black male was the victim of a single stab wound to his left side with subsequent hemorrhage within the chest and abdominal cavities secondary to perforation of the left lower lung lobe, diaphragm, stomach, colon, left kidney and clinically the spleen. He died approximately 11 days after his wounding.

At autopsy, anatomic findings confirm the above. No additional significant pathology was noted.

After review of the historic and autopsy findings contained herein, it is my opinion that the deceased, Reginald Daye, died as the result of complications of a stab wound to his chest.

Hardly a startling conclusion, given the description of the wound in the report.

In court the last time the defense attorney said that the knife blade was wide and the small wound would not be deep enough to cause that much damage. From the autopsy it is obvious that the knife blade was small and long, not fat. More like a boning knife and less like a carving knife. I am surprised that the autopsy did not indicate the type of knife. Maybe that will be in a different report.

Complications from a "gaping" stab wound led to the death of a Durham man in April, according to an autopsy report released Friday.

Crystal Mangum, 32, the Durham woman who falsely accused three Duke University lacrosse players of rape in 2006, is charged with murder in the man's death.

Police have said that Reginald Daye, 46,was stabbed with a kitchen knife during an April 3 argument at his apartment, at 3507 Century Oaks Drive. He died 10 days later.

According to the autopsy report, the stab wound punctured Daye's left lung and tore through his diaphragm, stomach, colon, left kidney and spleen. Daye also had what appeared to be defensive wounds on his left arm, the report states.

Daye and Mangum had been dating for about a month at the time of his death, but his family members said they weren't a couple. They said Daye was only trying to help Mangum get back on her feet and had provided her a place to stay.

In March 2006, Mangum claimed three white players on the Duke lacrosse team trapped her inside a bathroom at 610 N. Buchanan Blvd., where she was performing as a stripper at a team party, and raped and sexually assaulted her.

Her story about the incident was so inconsistent that state officials later declared the players innocent, saying there was no credible evidence against them.

Can some of the medical experts help me out on this? From my reading of the report and from what i know of the events.

1. He walked down to his nephews apartment then walked back to his own apartment which is where the ambulance picked him up.

2. The autopsy report listed all the organs damaged by the knife and the fact they were all sewn up.

3. He had almost a quart of "bloody fluid" in his abdomen.. Is that regular? Did they possibly miss something or not do a good job sewing up the damage?

4. If he was unconsious at the hospital did he bleed out? Lose blood pressure supplying his brain? Did he go into a coma from lack of oxygen to his brain and died from brain damage? Nothing abnormal noted in the autopsy to the brain?

5. Other than "complicaions from a stab wound". What broke? Why did he die? Shock? Blood loss? What?

His heart failed and the ventricles stopped pumping blood even though there was still measurable electrical activity in the heart, he was also throwing up (emesis). The heart problem was most likely secondary to his stab wounds. His brain went without oxygen because his heart was not beating correctly, if at all, for about 20 minutes. It is very difficult to recover from such a massive stab wound.

"Per chart note, on the evening of April 6, he was noted to have oxygen desaturation in the setting of emesis. He then experienced a pulseless electrical activity arrest and required 20 minutes of Advanced Cardiac Life Support (ACLS) prior to regaining spontaneous circulation. Subsequently, he was noted have a Glasgow Coma Score of 3, and cooling protocol was initiated. He did not regain meaningful neurologic function."

Good questions.

1. The immediate damage apparently was not sever enough to cause him to stop functioning. Although it would have been better if he stayed quiet.
2. organs can be sewn up but that does not mean they are all working correctly. There is still damage and fluid leakage can occur. Try sewing up a sponge (lungs) and then using it. Does not work quite the same as an intact sponge. Same for other organs.
3. Fluid accumulation in the areas of the wounds would be normal. Think of a scrape on your arm that oozes for a period of time Change the bandage. In this case drain the area. The fluid could also have accumulated after death. We all have some fluid inside us but our normal physiology prevents it from accumulating.
4. He died because I believe they pulled the plug. He was brain dead and could not perform any life surviving functions on his own. He was in a non recoverable coma due to lack of blood circulation and oxygen deprivation to his brain and other organs because his heart as not beating.
5. He died because stab wounds to the interior organs were sever enough to stress the body (heart) sufficiently to cause it to not function the way it should have.

The Durham man whom Crystal Mangum is accused of killing in April died of a stab wound that punctured six of his internal organs, according to an autopsy report released Friday.

Mangum is accused of using a knife to stab Reginald Daye, 46, in his apartment at 3507 Century Oaks Drive during a domestic dispute April 3. He died April 13 at Duke University Medical Center. Daye and Mangum had been dating at the time.

The single stab wound that led to his death punctured his left lung, diaphragm, stomach, colon, left kidney and spleen. The wound left what the report calls a “gaping” hole. He also had several cuts and bruises on his left arm, which might be from self-defense attempts, said the report.

Mangum is the woman who accused members of the Duke lacrosse team of sexually assaulting her five years ago while she worked for an escort service. The chargers were eventually dismissed by state Attorney General Roy Cooper when they were determined to be false.

So basically he died on the 6th, but they kept him alive on machines until the 13th. That really makes much more sense to me now.

Is there any indication of the time of day he was admitted originally on 4/3?

the size and length of the knife that Mangum used to stabbed Daye.

And for the Judge to simply accept that characterization of the knife,
and to subsequently question in open court how a person died from
such a stab wound BEFORE he even had the autopsy report!!!???
Is this a keystone cops based court system?

Syd should just keep on working on principles of restorative justice
for being escorted off Duke's campus.

I dont think the prosecution knew the size of the knife so she could not dispute the defense claim. But she could have tried.

The claim that there were multiple knife wounds was obviously mistaken. In the excitement of battle it appears that mangum made a few attempts at stabbing Daye and he fought them off as indicated by his defensive wounds. Mangum has quite a powerful arm swing to leave bruises so large on the Dayes arm.

I believe Daye was stabbed about 3:15 am on the morning of April 3 and taken to the hospital shortly after that.

He then experienced a pulseless electrical activity arrest and required 20 minutes of Advanced Cardiac Life Support (ACLS) prior to regaining spontaneous circulation. Subsequently, he was noted have a Glasgow Coma Score of 3, and cooling protocol was initiated. He did not regain meaningful neurologic function."
On the evening of April 6th, Day vomited and then experienced oxygen desaturation. Did he aspirate on what he vomited. IOW, did the vomitus end up in his lung which caused the oxygen desaturation?

"Aspiration of vomitus-- the inhalation of regurgitated gastric contents into the pulmonary system"

Can some of the medical experts help me out on this? From my reading of the report and from what i know of the events.

1. He walked down to his nephews apartment then walked back to his own apartment which is where the ambulance picked him up.

2. The autopsy report listed all the organs damaged by the knife and the fact they were all sewn up.

3. He had almost a quart of "bloody fluid" in his abdomen.. Is that regular? Did they possibly miss something or not do a good job sewing up the damage?

Apparently not. If there was any pathological aspiration leading to a stoppage of breathing it would have been mentioned. Vomiting is one reaction that can happen when there are heart stoppage problems. The problem with the heart was the heart was not pumping any blood. So no organs were getting any nutrients (including O2) during that 20 minutes. The heart problem can be caused by trauma. A deep stab wound affecting multiple organs is trauma.

ETA: the way the report is written it appears that the heart problem occurred first and vomiting occurred second. But the events were almost simultaneous.
CORRECTION: rereading the report it appears he stopped breathing and vomit was present, and then it was noticed that his heart was not pumping. Since there was no comment about aspiration one cannot conclude that aspiration of vomit occurred.

I dont think the prosecution knew the size of the knife so she could not dispute the defense claim. But she could have tried.

The claim that there were multiple knife wounds was obviously mistaken. In the excitement of battle it appears that mangum made a few attempts at stabbing Daye and he fought them off as indicated by his defensive wounds. Mangum has quite a powerful arm swing to leave bruises so large on the Dayes arm.

I believe Daye was stabbed about 3:15 am on the morning of April 3 and taken to the hospital shortly after that.
Good point -- I was wondering what the prosecutor's response was
when the defense lawyer was making claims about the size of the knife.

I wonder if the defense lawyer was only going by what Mangum told her
about the kind of knife she grabbed to stab Daye.

Good point about the number of stab wounds as well. IIRC, I think
the initial reports varied from implying a single stab wound to stating
something like 7 stab wounds.

She must have been in one hell of a fit of anger to stab him so fiercely like that.

good guess. The defense is probably running around trying to find bail before the judge increases it again due to the defense argument from last time not holding any water.

Just curious how long after he was attacked did the doctors begin treatment..

They posted that Reggie was up walking around and about to come home.

The truth is.......he was dead within 3 days of being admitted to the hospital.
