However EXTREMELY unlikely, I think it would be interresting to find a planet other than Earth where humans have evolved, as well as to see the lifeforms that would live there.

Well if you are a believer in alternate dimensions then certainly it is possible. The Speculative Dinosaur Project follows something similar to that, in which humans from our own Earth are visiting an alternate world. There is also a series of novels in which a Neanderthal scientist from an alternate world creates some sort of dimensional portal and ends up in our own world.
