Only major cities have kings, and I'm pretty sure they all answer to Ba Sing Se's.

None of the nations are directly related to anything in the real world, though sometimes they do have obvious inspirations (The Sun People are Native American's, for example).

The Fire Nation is Imperial Japan-esque but it has a lot of elements of Germany and some phases in America's history. Basically it's racism combined with imperialism in the most negative ways.

Earth nation is kind of like Feudal Europe. I don't see to much American in it (although I can't help but identify the two for some reason)

Air Nomads: Mostly Eastern Budhists monks, but have some behavior from a lot of native people's the world over.

The Water tribes from the North/South: The closest to a direct link to the real world is the southern, being obviously Inuit. The North is kind of like a fantasy ice city you would imagine, but very sexist against women.

Unfortunately it is.

To contrast, Aang giving a speech to Earthbenders in a camp COVERED IN EARTH in the movie to what really happened in the show.

In the show Katara gave the speech and the Earthbenders were in a metal ship in the middle of the ocean. All they had was coal to bend.
